지난해 재난적의료비 1582억원 지원…역대 최다

국민에게 의료비 일부를 지원해 가계 파탄 방지를 위한 제도인 재난적의료비 지원 규모가 지난해 역대 최다를 기록한 것으로 나타났다.

보건복지부와 국민건강보험공단은 2024년도 재난적의료비 지원사업의 실적이 대폭 증가했다고 밝혔다.

재난적의료비 지원사업은 가계 소득수준에 비해 과도한 부담이 되는 재난적의료비가 발생한 경우, 의료비 일부(80%~50%)를 지원하는 제도이다. 2018년 7월부터 입원은 모든 질환, 외래는 부담이 큰 중증질환을 중심으로 의료비를 지원했으나, 2023년도부터 대상 질환, 재산기준, 의료비지원기준, 산정기준 등 개선을 지속 추진했다.

2024년도에 집행한 재난적의료비 지원 건수는 5만735건(전년대비 51.1%증가), 금액은 1천582억원(전년대비 56.6% 증가)을 지급해 역대 최고를 기록했다.

연도별 지원 금액을 보면 ▲2020년 340억7600만원(1만3476건) ▲2021년 446억4300만원(1만6913건) ▲2022년 601억5200만원(1만9753건) ▲2023년 1010억1600만원(3만3585건) 등 꾸준히 증가하고 있다.

전체 건당 평균지원금액도 2023년에 비해 3.7% 증가(301만원→312만원)했다. 세부적으로는 기준중위소득 50% 이하 대상자에게는 건당 평균지원금액은 4.4% 증가(251만원→ 262만원), 질환별(암, 뇌혈관질환, 심장질환, 희귀질환 등 7개 질환)로 구분하면 중증질환에 대한 건당 평균지원금액은 2.8% 증가(389만원 → 400만원)한 것으로 나타났다.

2025년도 재난적의료비 지원사업의 예산은 1천424억원(일반회계‧복권기금 등 국비와 건강보험 재정으로 지원)으로 편성됐다. 2024년 관련 총 예산은 1천606억원(연초 1천335억원+271억원 추가 편성)이었다.

보건복지부 권병기 필수의료지원관은 “재난적의료비 지원사업은 과도한 의료비로 경제적 어려움을 겪는 국민을 보다 두텁게 보호하는 것이 목적이며, 지원의 사각지대가 발생하지 않도록 제도개선을 지속하는 등 노력하겠다”라고 밝혔다.

한편 재난적의료비 지원 대상은 기준중위소득 100% 이하(100%~200%는 개별심사), 재산 과표 7억원 이하로, 본인부담 의료비가 가구 연 소득 10% 초과한 경우 환자 또는 대리인이 국민건강보험단에 신청하면된다.

지원항목(1만원 미만의 소액 진료비 및 단순약제비, 미용·성형, 특·1인실 병실료, 간병비 효과 미검증 고가치료법 등 비필수 의료비 지원 제외)은 비급여 및 본인부담상한제 미적용 급여로 소득에 따라 의료비 본인부담금의 50%~80%를 연간 최대 5천만원 한도에서 지원한다.
