내달부터 트랜스티레틴 아밀로이드성 심근병증 치료제 ‘빈다맥스캡슐’ 건보 적용

다음 달 1일부터 트랜스티레틴 아밀로이드성 심근병증 치료제 빈다맥스캡슐(성분명 타파미디스)에 건강보험이 적용된다.

보건복지부는 27일 오후 제4차 건강보험정책심의위원회(이하 건정심)을 열고 이같이 의결했다. 

이 질환은 트랜스티레틴 단백질이 불안정해지면서, 심장에 아밀로이드가 비정상적으로 축적되어 심장 근육의 장애를 일으키는 희귀질환이다. 비가역적 심장 기능 손상을 유발해 심부전·신장질환·간질환 등을 동반하고, 심한 경우 사망에 이른다.

화이자의 빈다맥스캡슐은 식품의약품안전처에서 허가된 트랜스티레틴 아밀로이드성 심근병증의 유일한 치료제다. 트랜스티레틴을 안정화해 아밀로이드 생성을 억제하고 환자의 심혈관계 입원율 및 사망률을 낮추는 등 임상적 유용성이 있다.

해당 질환은 희귀질환자 산정특례 대상이다. 산정특례 대상자가 빈다맥스캡슐을 사용 시 본인부담금은 요양급여비용 총액의 10%가 적용된다. 치료제 연간 1인당 소요 비용은 3천650만 원인데, 희귀질환 산정특례 적용 시 약 365만 원으로 줄어든다.

보건복지부는 “중증·희귀질환·백혈병·항암제 등 환자에게 필요한 신규 약제는 급여화하고 기존 약제는 사용 범위를 넓히는 등 보장성 강화를 지속 추진하고 있다”라며 “이번 치료제의 건강보험 적용을 통해 환자와 그 가족이 치료에 대한 경제적 부담을 덜길 바란다”라고 밝혔다.
