고난이도 소아 수술 가산항목 확대…6세~16세 소아청소년 가산도 신설

정부가 소아에 대한 고난이도 수술에 대해 보상을 늘리고, 부인암에 대한 진료 보장성을 강화하기로 했다.

보건복지부는 27일 오후 2025년 제4차 건강보험정책심의위원회를 열고 이같이 의결했다. 

이에따라 올해부터 고난도 소아 수술 가산 항목이 확대되고, 6세 이상 16세 미만 소아청소년 대상 가산도 신설된다. 6세 미만 고난도 소아 수술행위를 추가해 기존 284개에서 603개로 가산한다.

또 대상 나이를 6세 이상 16세 미만으로 확대해 487개 항목의 100% 가산을 신설할 예정이다. 단계적 수술이 필요한 질환 특성이 반영될 수 있도록 급여기준도 개선하기로 했다.

관련해 복지부는 지난해 5월 소아 고위험·고난이도 수술의 연령 가산을 ▲1.5kg 미만 300%→1000% ▲신생아·1세 미만 200%→400% ▲1세~6세 미만 200% 신설 등으로 확대한 바 있다.

또한, 이날 건정심에서 부인암 진료 보장성 강화도 함께 의결됐다. 이에 따라 자궁경부절제술의 보상수준을 강화하고, 비급여 유방암 디지털 단층촬영술을 급여로 전환된다.

자궁절제술은 복강경이나 개복을 통해 암 조직을 포함한 자궁과 자궁경부 등을 광범위하게 절제가 시행된다. 하지만 자궁경부암 초기에는 경부 부위만 절제, 자궁체를 보존해 가임력을 유지할 수 있다. 정부는 이러한 ‘광범위 자궁경부절제술’ 행위목록을 신설하고 보상수준을 강화하기로 했다.

그간 젊은 여성이 자궁경부암에 걸리면 자궁 전체 적출이 아닌 자궁경부 절제를 통해 가임력을 유지할 수 있으나 행위분류가 없어 낮은 수준의 수가가 적용됐다. 앞으로는 ‘광범위 자궁적출 및 양측골발림프절절제술’의 121% 수준으로 보상이 이뤄진다.

아울러 유방암 진단은 진단 정확성과 병변 발견율 향상에 따라 초음파, MRI 대비 비용효과성에서 장점이 있는 디지털 단층영상합성촬영술을 비급여에서 급여로 전환될 예정이다.
