의료계 "ADHD 치료제 사용이 마약 입문? 사실 아냐"

대한정신건강의학과의사회가 ADHD 치료제가 약물 남용 위험이 감소하거나 변화가 없다는 장기 연구를 들어 일각에서 제기된 “ADHD 치료제가 마약으로 가는 입문 경로가 될 수 있다”라는 주장을 정면으로 반박했다.

의사회는 “ADHD 진단을 받고 의사의 처방에 따라 치료제를 복용한 환자들이 향후 불법 마약, 알코올, 담배 등의 물질에 더 많이 노출된다는 근거는 없다”라며 “일부 연구에서는 적절한 치료가 향후 약물 남용 위험을 낮춘다는 보호 효과까지 보고된 바 있다”라고 설명했다.

관련해 UCLA 연구진은 2천500명 이상의 ADHD 아동을 수년간 추적한 결과, 치료제 복용 여부와 향후 알코올, 니코틴, 마리화나, 코카인 등의 사용 위험 사이에 유의미한 차이가 없다는 사실을 밝혀냈다.

스웨덴의 국가 코호트 연구에서도 ADHD 환자 중 치료제를 복용한 이들이 복용하지 않은 환자보다 향후 물질 남용 위험이 31% 낮은 것으로 나타났다. 스웨덴은 복용 기간이 길수록 보호 효과가 강해졌다고 밝혔다.

의사회는 “ADHD 약물과 게이트웨이 드럭 개념은 다르다”라고 밝혔다. ‘게이트웨이 드럭’(Gateway Drug)이란 용어가 마리화나 등 비교적 약한 약물이 강한 마약으로의 입문 경로가 된다는 가설에서 유래했지만, ADHD 약물에 이 개념을 단순 적용하는 것은 과학적으로 정당화되기 어렵다는 것이다.

이어 “ADHD 자체가 충동성, 위험 행동, 환경적 취약성을 동반하는 질환”이라며 “치료받지 않으면 오히려 물질 남용 위험이 증가할 수 있다”라고 지적했다. 의사회는 “위험은 약물 자체가 아니라 치료받지 않은 ADHD 그 자체에 있다”라고 강조했다.

그러면서 “ADHD 치료제에 대한 불필요한 공포와 오해를 해소하고, 정확한 정보에 기반한 치료 기회가 보장되어야 한다”라며 “ADHD 약물은 치료를 위한 도구이지, 마약의 문이 아니다”라고 덧붙였다.
